We welcome new pateints and offer convenient appointment.
You can expect a friendly experience when you arrive at our office.
Please arrive at least 30 minutes before your scheduled appointment.
Please bring your insurance card. We will need a copy of it for our records. If your insurance plan requires that you have a referral from your primary care doctor, please make sure you obtain one before your appointment.
Print and bring in your completed New Patient Forms. This step will save you time on your initial visit!
Please fill out and sign all new patient forms. Keep Patients Privacy Notice for your reference. Bring all signed forms to your appointment.
To ensure a positive care experience, visits to Stride Foot and Ankle Center are made as comfortable and convenient as possible. There’s plenty of free parking, and our easy-to-find office is handicapped-accessible.
If at any time you are unable to keep your appointment, please notify us at least 24 to 48 business hours in advance. We will be glad to reschedule your appointment at a more favorable time. We greatly appreciate your time and consideration and look forward to seeing you.
