We always welcome new patients and try to offer convenient appointment times. We guarantee a friendly encounter from the second you enter our office. We ourselves value being on schedule and accordingly request that our patients respect the schedule and show up before their appointed time.
We require that patients arrive at least 30 minutes before their appointment time.
Please bring your insurance card. We will need a copy of it for our records. If your insurance plan requires that you have a referral from your primary care doctor, please make sure you obtain one before your appointment.
Print and bring in your completed New Patient Information Forms. This step will save you time on your initial visit!

Click on the links below to access the New Patient Form. After you click the link, a browser window will open. After it opens, click on the ‘PRINT’ icon on your Browser. After the form prints, fill it out and bring them into our office.
Please fill out and sign all new patient forms. Keep Patients Privacy Notice for your reference. Bring all signed forms with you the day of your appointment.
To ensure a positive care experience, visits to Stride Foot and Ankle Center are made as comfortable and convenient as possible. There’s plenty of free parking, our easy-to-find office is handicapped-accessible.
If at any time you are unable to keep your appointment, please notify us at least 24 to 48 business hours in advance. We will be glad to reschedule your appointment at a more favorable time. We greatly appreciate your time and consideration and look forward to seeing you.

We accept most insurance plans. Please call us to verify whether or not we are participating in a particular insurance company or visit / refer to our insurance page .
We also accept in office cash payment and payment plan can be arranged. Please contact the office for more details or if you have any other questions.
All CO-PAYS are due at time of service.
